Sage, M.D. |

Dr. Sage was born
and raised on the west side of Chicago. He is a graduate of
Loyola University Stritch School of Medicine. He did residency
training in Internal Medicine at St. Elizabeth's Medical Center
in Brighton, Massachusetts and Family Practice at Lutheran
General Hospital. He is board certified in both Family Practice
and Internal Medicine. He has been involved in many community
activities. He was a charter member of the board for the
Lutheran General Health Plan, the first medical director of
Rainbow Hospice where he served for fifteen years and now is
associate medical director. He is a member of the Arlington
Heights Smoke Free Coalition. Dr. Sage was the honoree at the
Rainbow Hospice Gala in 2001 and was chosen as Family Physician
of the year 2002 by the Illinois Academy of Family Physicians. A
long term resident of Des Plaines, Dr. Sage and his wife,
Elaine, currently raise their two children in Arlington Heights.
Dr. Kragt was born
and raised in Iowa and now lives in Mt. Prospect with wife,
Joleen, and daughters, Natalie and Jaclyn. He is a graduate of
the University of Iowa Medical School and completed a Family
Practice residency at West Suburban Hospital in Oak Park,
Illinois. He is board certified in Family Practice and is on the
attending staff at Lutheran General Hospital and Northwest
Community Hospital. Dr. Kragt has a strong interest in
preventive services, cardiovascular disease, diabetes and sports
medicine. He also performs no-scalpel vasectomy. His time
outside of medicine is filled with family time with his girls
and playing basketball.
Dr. Gallup was born
in Iowa and graduated from the University of Illinois Medical
School. He is a graduate of the Family Practice residency at
Lutheran General Hospital and is on staff at Lutheran General and Northwest Community
Hospitals. He is board certified
in Family Practice. He and his wife Sandi are raising their
two daughters in Mt. Prospect. Dr. Gallup was formerly a
chiropractor and registered respiratory therapist and has served
patients since 1980 and has strong interests in sports medicine,
weight control, diet and exercise programs.
Dr. Thompson was
born in Dublin, Ireland and graduated from the University of Dublin and
did residency at St. Michael Hospital, Dublin. He has been in
private practice serving families in the northwest suburbs since
1973 and is on staff at Lutheran General Hospital. He is the
office resource for French speaking patients and has a strong
interest in adolescent medicine in addition to health screening,
treatment and preventative services for families. He and his
wife Shelia have three adult children.
