                                                              APPOINTMENTS

     Appointments are made with the receptionists. Individual problems require different lengths of time
     to evaluate and treat you properly. It is important for you to accurately describe your problems so
     that the appropriate amount of time can be scheduled. Please let the receptionist know if you are
     coming in for more than one problem. We request that new patients have a comprehensive history
     and physical examination at their first visit or soon thereafter. This will allow the doctor to become
     better acquainted with you, to review your family's medical history, to identify current health
     problems, and to include you in the development of a plan for your health care. The new patient
     comprehensive visit is generally one hour and may include laboratory tests, which will be discussed
     at the end of your visit. Please try to schedule routine (check-up) appointments several weeks in
     advance to get the day and time you desire.

            

     If you find it necessary to cancel an appointment, please do so 24 hours prior to your scheduled
     time or you may incur a cancellation fee. We understand that there are always exceptions and
     patients also experience emergencies. If you miss your appointment without having cancelled your
     scheduled time, it is considered a "No Show" and you will be sent a reminder of our policy. If you
     should "No Show" a second time you will be charged a fee of $50.00. A third "No Show" could
     result in the patient/physician relationship being terminated as well as a second "No Show" fee.

     We hope that you will be on time for your appointment. This allows us to be efficient and to spend
     the maximum amount of time with you. It also minimizes your waiting time and that of other
     scheduled patients.

                                                              OFFICE HOURS    

     The office opens at 8:00 a.m., Monday through Saturday. At this time, our staff will be available to
     answer the phone, take messages for the doctors, schedule appointments, and perform certain
     laboratory tests. At least one of the doctors will be available by appointment as follows:

                                       Monday            
  8:00 a.m. - 7:30 p.m.
                                       Tuesday            
 8:00 a.m. - 7:30 p.m.
                                       Wednesday       
 8:00 a.m. - 7:30 p.m.
                                       Thursday           
 8:00 a.m. - 7:30 p.m.
                                       Friday                 8:00 a.m. - 7:30 p.m.
                                       Saturday             8:00 a.m. - 12:00 p.m.  (For emergency problems only)


                                                    EXTENDED CARE COVERAGE  
    
     If you or a family member require care in an extended care facility, we will be glad to discuss with
     you the facilities in the area that we service.

                                         ILLNESS / QUESTIONS FOR THE DOCTOR     

     For routine calls, a message will be taken and the call returned later in the day by the physician or
     their nurse. For more acute problems, please explain the urgency to the receptionist / scheduler.
     She will notify the doctor and they will return your call as soon as possible. In either case, it is
     necessary to leave your message with our receptionist / scheduler. You may also ask to speak
     with the triage nurse who is available for your urgent questions. For acute illnesses or injuries,
     calling early in the day will allow us to see you that same day. Please try to keep your phone line
     free for the return call, have your pharmacy number ready in case medication needs to be
     prescribed, and have a pen and paper to write down any instructions.

                                              NON-OFFICE HOURS EMERGENCIES 

     Our telephones are answered by a 24-hour answering service when the office is closed. They will
     take your message and notify the doctor on call. He will return your call as soon as possible. For
     extreme emergencies, dial 911. They will contact us after stabilizing the situation. You should
     keep the office number, and the number of the poison control center near the telephone for family
     members and babysitters. No narcotics or controlled substances will be filled after hours.
     This is not the time to call for routine refills or follow up questions. Antibiotics will only be given
     after hours in extreme cases which do not allow time for an office visit to be scheduled.

                                                               LAB RESULTS

     We will notify you regarding your laboratory test results either by mail, (if normal), or by phone call,
     (if abnormal). Please do not call us for results unless an unusual amount of time (over one week)
     has passed or you have been requested to do so.
